As students begin to return to our buildings, we are taking many steps to keep everyone safe.
If you are sending your child to school, please remember to have
this student health form completed. If you already completed this form earlier in the year, you do not need to complete it again. If you haven’t completed this online form, please take a few minutes today to complete it for each of your children who are attending in-person instruction.
If your child is continuing with virtual instruction, you do not need to complete this form.
